View 2021 Enable Midstream Benefit Rates
You have two medical plan options: a High Deductible Health Plan (HDHP) paired with a Health Savings Account (HSA) and a PPO Plan. Read on to learn the differences between the two options. Blue Cross Blue Shield of Oklahoma (BCBS) and Express Scripts are our plan administrators for our medical and prescription drug plans.
You can choose the right level of coverage for you and your family — Employee Only, Employee + Spouse, Employee + Child(ren), or Employee + Family.
Enable Midstream's medical plans are self-funded. That means that while claims are processed and paid by BCBS and Express Scripts, the money actually comes from Enable Midstream's bank account. Enable Midstream and you share costs for coverage and care through premiums, copays, coinsurance and HSA contributions. Throughout the year, we will encourage you to use these benefits as wisely as you use other Enable Midstream resources. Using generic drugs instead of name-brand, getting regular preventive care and creating a relationship with a good doctor are all ways to use your medical benefits wisely.
If you are (or another covered individual on your medical plan is) receiving care from a doctor or medical facility who leaves the BCBS network for reasons other than misconduct, breach of contract, loss of license or a similar reason, you may be able to continue paying in-network costs for services from that doctor or medical facility if someone on your medical plan is receiving ongoing treatment for:
Full-time employees who work a minimum of 30 hours per week are eligible for benefits. New full-time employees scheduled to work at least 30 hours per week become eligible for benefits on the first of the month after their hire date. New hires must enroll within 31 days of their hire date.

Enable Midstream reserves the right to verify dependents at any point. Please make sure you are only covering eligible dependents. If you are found to have covered an ineligible dependent, you may be responsible for paying back the cost of medical services covered by the plan for the dependent and costs of premiums paid by Enable on behalf of the dependent, and he or she will be removed from coverage immediately.
The choices you make during enrollment will remain in effect through Dec. 31, 2021, unless you experience a qualifying life event such as marriage, divorce, birth or adoption of a child, or gain or lose coverage through another insurer. You'll have 31 days from the date of the life event to make benefits changes that are consistent with the life event (e.g., add child coverage when a child is born, drop spouse coverage when you divorce).
